SBI does give statement on request on dot matrix paper. During my initial entry to UK I have submitted bank statements from SBI in dot matrix with stamp and signature. Try to find any known person in that branch or meet Manager and explain your problem hope they will give you the statements.

Yes Tier 1 dependent has right to work and there is no restriction of hours. My spouse is working in public sector full time permanent without any trouble and during visa extn she has submitted HO ref letter which was in my name. And it was accepted.

You dont have to prove your english lang. again as you have already proved that in your initial application. Tick apropriate option for your English language skills in the application.

Hi Yarlagadda, You have to attach pre paid Royal Mail SD next day envelope with your application and dont forget to note the tracking number so that you can keep tracking your return post. If that holiday payment is mentioned in your payslips and reflecting in your bank statement then you will be fine. But I have doubt if it is paid in cash cos its hard to prove. In that case you can attach employer letter.

For any Original documents you dont need sign and stamp. But if you are using one account for earnings and funds then mention it in your cover letter.
