If that holiday payment is mentioned in your payslips and reflecting in your bank statement then you will be fine. But I have doubt if it is paid in cash cos its hard to prove. In that case you can attach employer letter.

For any Original documents you dont need sign and stamp. But if you are using one account for earnings and funds then mention it in your cover letter.

Ad hoc bank statements are acceptable but to keep yourself safe side you can attach business card of the Bank Manager and also mention details in your cover letter.
