looks like you are eligible on the basis of the information you have given.

for question 2.1, just write down the date of the first time you entrered the UK on a non-visitor visa. (so as a student for you)

You just need to spend 10 years in any category. So if you switch to a work visa, you can still apply for ILR after 3 years rather than the 5 years as you will be eligible for the Long Residency ILR. My advice would be to try to switch to Tier 2 if you can, because you never know if the Long Residen...

a visa cannot be deactivated or activated...

in your case:

activated = valid visa
deactivated = cancelled visa.

his visa has been cancelled, to return he wll have to make a new application.
