Send more bills to prove your residency. You can also request HMRC employment history from HMRC covering last 5 years - it will show where you worked. P60s and few payslips are enough. The problem will be in the year you do not have P60 as more payslips would be advisable. The letter from HMRC might...

Send the missing documents together with cover letter explaining the situation to the same address you sent all other documents. Mention the case reference number or whichever Home Office numbers you have, and all details - date of birth, nationality etc. It might delay the application decision, but...

I doubt there will be any advice on this. 10 days are 10 days. Either "play the lottery" and hope it will arrive in time or change the flight dates - depending on which option will be chaper for you.

I would not warry too much about the name of the card as long as it is capable of making international payments. There might be a fee which is a different matter. You only resubmitted your biometrics so far. They did not ask you to resubmit your application and pay again. No panic than. Biometrics are taken after aplication submitted and payment taken so I would think there will be no fresh application and no additional fee.

Strictly speaking, no. But as you can travel to the UK without visa and stay for up to 6 months, there should not be a problem if you explain to immigration officer your situation and show him the police report.
