The 2006 version came into force from Monday 3/04/06, its called HSMP 1(Form version 04/06) valid from 03/04/2006 to 18/06/2006). I used this form as I applied in June 2006. I did fill a previous version of the form, HSMP1 Form Version 04/05 but never send it off for some reason, thats how I missed...
