If on his birth certificate his surname is May Sorensen, then you put that on the form. First name and middle names will be in 'other names'. you say he has got 2 surnames on the BC do you not mean he has 2 names as his surname. Have you considered putting a hyphen and making it 1 name ie May-Soren...
