Tier 1 employment letter

As part of my previous earnings I will be showing letter from the employer and bank statements. Since I also get accommodation allowance i am confused about the format of the letter.....anyone got template which i can use?

Thanks for your reply....another quick question...i have been reading the forum and lots of ppl are saying that immigration officers are calling part time employers asking for info....is tht in every case? i am gonna just work there for a month and its takeaway shop and ppl who i doubt would remembe...
