K44MYY that's the correct number and they will ask which city you want to book for. There is TB testing available in Mirpur. If you got a job then you will be ok. I believe three months is the absolute bare minimum but six months is highly recommended.

I'm only familiar with VAF4A but it clearly states in the guidance notes that other names includes names that you have been known by, in other words yes you should include a maiden surname here. I put 'maiden name' in brackets after it. Hope that helps.
