Birth certificate is not needed as you submit Passport as a age proof. A covering letter will help the case worker and ease the pressure on the applicant too. Depends on when you submit the application. If the date of travel is within 3 months of your application, Visa would start from October 1.

Yes, digital signatures are not acceptable. As per the guidance, payslips generated online would have to be stamped and signed.
