Well it make no sense at all, home office would be ridiculous to assume that applicants should book and pay for flight tickets before getting visa! as for date, sponsor is flexible on date of visit, not an issue for them in UK. Your details will still be in the NHS database. If changing NHS surgery give them details of your previous NHS surgery. The computer system should be able to trace you. Hi You dont need to worry, when you will send documents for ILR again just send the previous refusal copy with covering letter mentioning that you already submitted life in the Uk test certificate original with previous application. Why you did not appeal when they refuse your spouse ILR application...
