If you don't have the latest statement, a certified copy should be fine. Some banks do it and others don't, but the Barclays next to the Croydon office definitely do. Who do you bank with?

A new letter stating the salary as well would be helpful.

You can apply as a dependant. Take originals and copies of the documents needed for cohabitation.

As it doesn't have the company stamp and signature, to be on the safer side I would get them signed and stamped. Also if your employer can give you a letter stating that they are the payslips for the period .... etc. that would be good.

I reckon 3 months should be fine, but I would take 6 months just to be on the safer side. the local authority statement is because i work in a school and my paid holidays are slightly different to others.
