I dont think as per UKBA rules you need any insurance if you are sole trader.
If you have it, its good.

Personal Bank A/c should be ok.

You can use the same a/c if you like to show funds for yourself and your wife.

Contract letter is not important but its your wish not mandatory.

You need only provide statements for actual income you are claiming. If you were only paid once you need only provide one statement for that amount. If you are actually able to get statements that cover individual transactions (rather than all transactions in a given weeks month), they should be ok...
