If you look at the PDF requirement I think you will find the answer to your question which says that the bank statement if stamped and signed by the bank is acceptable by the home office .
what is the name of the document should one get from HMRC for last 3 years tax returns? like sa302 is there any name of the document should i ask from HMRC to show the tax.
Its generally not a required in the guidelines. But I would suggest that you maintain the balance till you get the decision for peace of mind and in case any verification happens.
all the best
If you think that your application should have the document that you want to send. Its better that you send the documents with a covering letter and your reference number. It will be worth a try and will give you peace of mind.
all the best.

I think what criteria you have used while applying first time you need the same criteria in the extension. Even if you use the PBC in UKBA website it will prove the same.
all the best.
maintenace of fund can be shown in two accounts i guess ( seniors / members please correct me in case i am thinking wrong). I hope both of your account are UK accounts. Try to prepare the next application that you can file as soon as you get back your application.

please be careful about the PSW as its getting over by april 2012 . so in case you are deciding to get back your passport before decision be careful about that aspect. i would suggest that wait for UKBA decision and if required dont travel from UK till you get your visa.
