it is not unusual - when there are delays they refund premium fee (£400) even if the visa granted on same day.

You don't need to take any action... why you need to contact them? after all its your hard earned money;

Fees will increase after 6th April 2015, If you are applying by post you would need to pay the new fees, However if you are applying in person at PEO you can book appoint before 6th of April and pay the old fees online.

You should be fine, as it says 12 months consecutive in 15 months period. you can show less than 12 months earnings if you could get enough points.

can the same esol entry 3 certificate could be used for naturalisation in April 2016?

There is no hard and fast rule on section 5 (Finances)

But it is good to demonstrate that you are capable of looking after yourself and family.
Savings account, mortgage, salary slips or current account (with reasonable funds) will be enough to prove your finances.
