It will be fine.
In UK, the salary payment usually shows the company name, however this is not true outside UK.

most likely your application will be returned as invalid.
You need to apply again with up to date documents.

Not required.
It says to book an appointment within 15 days, not to have an appointment within 15 days.
The letter clearly mentioned this information.

Its 31 days now.
What will happen if I made an out of time application now and that gets accepted? Does this overstay will become legal?
and in case of refusal, will it be refusal + 1 yr ban?

Please help to understand the timelines for getting BRP letters for Tier1 switching /extension for in-country applications done after 14th Dec 2010.

You can formally complain to Home Office if your application is more than 14 weeks old. Check the UKBA website for contact details for the same.

There is no way to apply for T1 G from outside UK. That route has been closed since 23rd Dec.
I guess you applied in-country (within UK). Not sure if new documents allowed in appeal. Otherwise, you can apply again with right documents.
