Is your solicitor posted your application or through PEO service ? If it is PEO, then they should have asked additional documents at that time right ? UKBA is correct in your second extension.
Tier-1 Initial - 3yrs
Tier-1 first ext - 2 yrs
Tier-1 second ext - 3yrs


Applicants who got 2 yrs intial visa ,i.e after 06-Apr-2010 will get 2years in 2nd extension.
Tier-1 Initial - 2yrs
Tier-1 first ext - 3 yrs
Tier-1 second ext - 2 yrs
