Indian Passport Surrender form

Can anyone please help.

In the Indian passport Surrender form,
There is field for Father's name and next row for Spouse Name.
Do I need to write Mother's name (Spouse of Father?) or my Spouse?

Please help.

original certificate

Dear all,
I would like to know, if, I use post office check and send service for my first passport, do I need to send original certificate or will they photocopy and return me the original back?

hi

In Harrow there were 58 new citizens and as others have said few were smartly dressed, few in jeans and t-shirts. There is no as such a dress code. Its upto individuals. Only thing lacking was the enthusiasm in taking the oath and singing the national anthem. 58 people but still it was so low pitch....

Thanks for replying. Just to update you, everything is sorted. I am living in USA (at time of application) and got UK citizenship (in USA).
Just for your information, each application is dealt with case by case bases.
