The SET (O) guidelines only ask for the pay slip and bank statement (showing that the pay is transferred to your account) to be within one calendar month on the day of application.

I have successfully used a letter from the employer which was about a month old.

Thank you both. This clarifies it. I was thrown off by the statement,

"ALL four boxes must be filled..."

but didn't realise that there are actually five boxes :)

Section 5 Set(O) Form

Hello,

I am filling the Set(O) form and have a query regarding Section 5, actually the four boxes after 5.5 related to SOC and pay rate. My CoS specifies my annual pay and weekly hours (37).

Is this how I can calculate the hourly rate?

Weekly_Pay = Annual_Pay/52
Hourly_Pay = Weekly_Pay/37
