yes, I believe.

Can i claim the gross salary including quarterly bonus deducted every month?

If its on your payslip and deposited in account, you can claim.

As sudesh sir said, you will be allowed even its one day left on your visa. I would suggest apply once you are back. Get your approval letter. Apply for wife visa after your approval. This will save one extension and few hundred quid while applying for your wife visa.

No need to submit marriage certificate again. First time they granted visa was after satisfying all the requirements and authenticating your documents. Just go by simple extension. Even dependent form doesn't ask for any such document if applied inside the UK

Yes. You should inform them it was refused. They have provided the space so you can write the circumstances and ruling also. If you have any documents you can attach that.

They have all information. There will be no harm in disclosing something they already knew.
