If he's bulgarian he can only work as self employed unfortunately. Or try to apply for a work permit under highly skilled if he was a uni degree and worked in his home country.

you could still be working as self employed. there's no restrictions on that. as in for the healthy insurance, you would have to ask the NHS and then if everything is fine, register with a GP

You could have applied for the blue card after one year of continuous legal work. But you can also apply for the citizenship after 5 years of staying here and your wife and son can apply as dependants. More information here
