Hi Houser. As I said before, you can apply for a blue card which gives you the right to work here with no restrictions under the basis that you have legally worked here for more than 12 months. If he's bulgarian he can only work as self employed unfortunately. Or try to apply for a work permit under highly skilled if he was a uni degree and worked in his home country.

Moreover, she can get the full working permit - blue card after graduating uni which would give her the right to work in the UK with no restrictions.

you could still be working as self employed. there's no restrictions on that. as in for the healthy insurance, you would have to ask the NHS and then if everything is fine, register with a GP

You could have applied for the blue card after one year of continuous legal work. But you can also apply for the citizenship after 5 years of staying here and your wife and son can apply as dependants. More information here
