Seems like SET(F) or SET(O) are the only option even though when you use www.gov.uk website and give answers to questions, it takes you to SET(O) form. If applying in June, I would say you can go max beyond from apr13 to mar14 as 3months+mar14=jun14

yes you can do so e.g. i submitted from 01-apr-2013 to 30-mar-2014 while applying on 09-may-2014 for 2nd extension and they accepted it.
