I enquired about that too and was advised by the NCS in my area that the form should be filled in married name and a note should be made on page 15, requesting to have certificate in married name.

If she didn't pay anny health contributions in Romania, she might not be issued the EHIC. And as far as I know, you can't apply for it by post.
