Premium service does not guarantee same day decision for all. In some cases, due to rush or when extra checks are required the decision gets delayed.

You should be okay if total leaves are not more than 180 in 5 yrs and there is no continuous 90 day leave period.

There is no requirement that states that you cannot be outside UK for more than a month because if that's true, there is no need for the 90 day rule
