Main applicant section on the dependant application is mandatory and cannot be removed, this should already be clear.

What he meant / Or what I understood by main applicant section was a copy of main applicant's passport and visa.

The nationality shown is correct. UKBA doesn't mention 'Indian' but 'INDIA' as the nationality. For the place of Birth, I think you can check with UKBA by sending an email but I feel they have a character limitation on the card and that's the reason you don't see the City and State mentioned in full.

You can always by chip and pin but it depends if UKBA payment systems are running on that day. You can take a banker's draft as that would guarantee payment even if the UKBA payment system's are not working.

As per my understanding, you might not need second extension but apply for first extension just before 2 weeks of expiry at the most.

The ILR decision depends on caseworkers discretion as he/she may/ may not discount this 87 day period.

The letter is from the accountant so tat makes it source #1.
You have your bank statement so that makes it source #2.

I said include salary slips because that could serve as a 3rd source because salary slip would not be from the accountant but your husband's company.

Book a single appointment within 15 days, it doesn't matter when the appointment is. The main point is to book within 15 days which does not imply that you need to give the biometrics in 15 days. If you book a PEO appointment, they don't need the barcode and would also return the letters you receive...
