You don't need to fill D9 unless your passport is unavailable.

D3: This is the Visa issuing date not the application date

D4: Is the number written in red on the right hand side, top corner

The period doesn't matter, as long as the main applicant has a valid visa. It hardly takes a month to get the applications processed from India.

I believe the name addition into your friends account is the best alternative suggested here.

This would also mean that your friend would also have to sign on your Tier 1 visa application form, allowing UKBA to conduct checks.

Typically you should get it within 4 weeks but in some cases the Biometric letter can take up to 6 weeks.
