You don't have to wait for the statement. When ready just print the online statement/transactions and get that stamped from the branch. Ensure all the supporting documents MUST not be older than 28 days.

Hi.

I included the previous address. Make sure you send it through recorded delivery and keep the proof of it. They will send you a confirmation email in couple of days time.

You do not need to send husband's passport as he will hand it over to VAC when he will go for biomatrics.

More information required. If your main salaried job is more than 18600 then just put 6 payslips and 6 bank statements. Please ensure the salary figure on payslips are matching the ones on statements. ECO may contact you or your employer to verify this information.

No Problem at all. If you are applying for a spouse visa then there is a question if you have even known by a different name or you can add your previous name in the maiden name section. You can also explain this in your covering letter. This is not an issue at all.
