There is no requirement for the child to have a British passport. As said, HMPO won't issue a passport in a different name to any other uncancelled passport the child has. He shouldn't have sent his BRP to the passport office. What made him think he had to do that? There is a dedicated address for BRP return, I believe it's on a letter you get when you receive your naturalisation certificate. You should not have completed the question in 1.3 as you have not been granted ILR.

Technically all PR cards and DCPR's are backdated; due to the nature of how PR works you can only apply for one once you have acquired PR so your PR date will always be some time in the past.

Unlike some countries, the UK does not require exit visas. You are free to leave on your own, and you can reenter on your own with a family permit.
