Your form is defiantly FLR (hsmp) tier1 http://www.ukba.homeoffice.gov.uk/sitecontent/applicationforms/workpermits/hsmp/flrtier1guidance.pdf It clearly states who should fill it. Dear you don,t need an immigration adviser. Most of them know nothing like people sitting in ukba. Ukba website is the best guide.
