Mistake in the latest SET O form 05/15

Hi guys,

Be careful. The form has been updated on 6 May and it has a wrong version in the footer as of page 76.
If you print it out, it will look like you put last few pages from the old form, because of this mistake.

It's very weird, she should not have asked the letter and it is not required.
