There are no rules stopping you from applying for 2 years for the pair of them in one go. The only problem is, your dad has been here before, so he has a better chance of getting his visa re-issued. Regarding english requirement, my masters degree was recognised in the PBS calculator and I used the same for english language in March 2014.

The employer letter I submitted had a table of monthly gross and annual salaries. As I receive electronic pay slips, letter also mentioned that the payslips are authentic and reflects the tabulated salary.

We received similar messages - 3 messages on the day of application and 2 messages after 5 days. There was a word "courier" mentioned in the last message. Passport arrived a day later. All in all, it took only 4 working days for issuing the visa.
