3 month is sufficient, i gave for 3 months only as it covers the maintenance requirement period of 90 days (though there is no explicit requirement for maintenance funds)

It is not a mandatory requirement and as you said it is not mentioned in the SET(O) form but caseworker may ask for it. Employment history is not needed if you have all the P60's for last 5 years

It doesn't have to be 2 years before the application but any period of 2 continuous years after the last leave was granted. But it is best if you give the 2 years just before the application as it demonstrates your current cohabitation status
