I personally think you might not be eligible as you had a break in your visa. However, best to wait for one of the gurus to confirm as I am just telling you what I infer from the rules.

Since you entered within 28 days of your EC you can apply 28 days before the EC date so 28 days before 18th April.

absences for dependant is not a problem as long as she has been on the visa for over 2 years and you have the co-hab docs you will be fine.

They want to consider your application again. I think you should go ahaed and enrol for your bio metrics and also ask UKBA what about your wife's bio metrics. Its possible that your details were not properly recorded and hence they have asked for it for the 2nd time.
