The child's visa will have the same expiry as your visa. You can't choose less time to suit you.

Because for your dependent it is a 3 year extension once the main visa holder gets ilr, as dictated by the Immigration Rules.

You cannot choose to pay for less, it is 3 years and there is no discretion.

You got ILR 12 years ago via Tier 1 General based on your posting history.

Of course UKVI will expect to see updated valid proof you meet the English language requirement for Citizenship if you got ILR before 13 October 2012, which apparently you did as your PEO appointment was on 8th October 2012.

It's for surname/last name only.

The question before or after that is likely the Given or Other names question for first, middle etc names.

Also, you should be applying online and not using the paper form..

Once their visas expire they are overstayers.

This doesn't impact a UK born child's entitlement to register as British though.

NHS use though, is not free once their visas expire.
