Yes.
If yes, do I have to file an extension after 3 years to pay IHS until ILR is received which is after 5 years?
The child doesn't need ILR. The child can be registered as British once either parent is granted ILR.

It is not about section 3c, which extends your leave while an application is pending if your visa expires. This is completely separate to qualifying for ilr. What does your PR card say exactly? If you got PR before brexit, that card is no longer valid regardless of the expiry date and doesn't give you legal status, which is why it says no right to work.
