If you don't have a pin code, you can go to your bank with your card and cheque book alongwith proper identification and ask them to issue you a banker's draft payable to Home Office UKBA.

Banker's draft in lieu of payment is accepted by UKBA.

ajs, are you trying to book a family appointment or a single appointment. I booked a family appointment and these appointments are hard to come by and are taken instantly as they are released.

You might be able to book a single appointment for mid May.
