Can some one help me dealing with point B26 which says Employed or Self Employed.

Because I am both Salaried and Self Employed! Can I tick both the boxes or will that be either of them?

I have already spoken to my accountant who has agreed to give me a letter which clearly states that NIC can be paid after receiving the statement.
