tbey are not always available. U get a certain quota for each year, requests of which are made in Jan-feb time generaly, following an email from ukba. Normally the visa is given for time on the cos plus 2 weeks.
U can continue to work for same employer until end of the visa, and can apply for an extension any time before your leave expires.

as i have said before, it depends on cos.
if the employer has not given u this info, then ask them to give u a print out of cos, which should state the end date of the job/cos

it depends on how many more years u need to apply for. In your case it should be 3 years, as u will go pass the 5 years mark with this extension. however, this is not in your control. she can apply for new visa from india.
theoratically speaking, she should be on same dependent visa on whose main visa u are, so can be questioned at airport, once they enter ur previous cos and see it is not valid anymore

Its ok. What you could do is carry with u a copy of emails from employer notifying u the delay of start dates and new dates, as you may get asked by immigration officer at airport.

you can apply for 3 years visa even if your passport is valid for 3 months..
the new visa is not going to be stamped on passport, but will be a BRP (card), once u have the visa, u can renew the passport and use the new passport with BRP to enter exit UK.

either ur a rated employer has tick-marked for your maintenance or you need statement for £900 for your self + £600 for each dependent. It can be from any bank, if its uk bank, even better.

you might be choosing a specific location, which may not be offering tier 2 applications at that centre (my guess), due to any reason (refurishment of centres),
best try for searching appointments in other centres and travel for appointment.
