Does it really say "transparent"? I dont remember it did.
I simply cut a normal sized envelope into half, put tape on the size and used it. Worked for me.

My letter said exactly the same thing and i had the questions but i received my BRP 2-3 days after the approval.
Dont worry, i think this is the format that they have these days.

You only need to tick the section relevant to you. There is also the subtle technicality that once you are on ILR then basically you are not on Tier1 and thus child is not technically a Tier 1 dependant.
