It depends on what basis you are applying the passport.
If you are applying on the basis of naturalization then 2 and 3 blank are fine. You will still need full name of your mother although I don't think it will be an issue.

No, there are separate fields for the current address.
Same for MN1. I am guessing your daughter is only a couple of years old, if so then in general dont worry too much about MN1 application as they are quite relaxed for young children - this is just based on personal experience.

Yes you can leave it blank.
If the date is not relevent to your application then there is no need to write it. I was in the same situation and I simply left it blank, did not explain anywhere and it all went smoothly, they did not say a single thing in the interview or anywhere else about it.

This might not work for everyone but I simply did around 15-20 online practice tests, made a note of the ones I got wrong and then remembered those. Passed the test without any issues :D
No need for the book at all IMHO.
