Contractor covering letter for HSMP Extension

I have worked in two contract positions and now in permanent positions.

Since I was using umbrella company for invoicing, I was their employee. How do you mention the this in the covering letter? Do I have to say I was contracting? OR do I say I was employed with the umbrella company?

Has anyone doing contracting established their own company AND applied for HSMP extension ?

If yes, can you provide some information about things to do and the documents to be submitted when applying for extension.

HSMP Extension period

Hi, I have met lot of people who have come to UK on HSMP... very encouraging :D And like applying for HSMP, the HSMP extension has its own myths. I understand the normal extension period is 3 years. But I have heard the extension given periods varying from 1 year to 5 years. Is this true / possible ?
