Yes though they were holidays you would still need a letter from employer. It is a mandatory requirement for WP/Tier 2 holders.

You are eligible. All you need is a letter from your employer stating the absences were authorised paid annual leave. There is a template in the sticky note on this page

Your contract should have details about the notice period. Normally you have to abide by what it say on your contract. Generally it's 4 weeks if you have worked over an year

If money is not an issue for you, would recommend going for a PEO appointment just saves you time and the harassment in contrast to applying via post

As you haven't put any dates on it that when you will be applying for your ILR or if you have already booked an appointment. You should apply for SAR report without delay. Though they say 40 days, it can take(normally takes) upto 2-3 months.
