Yes, you need to apply for an extension. Yes, BRP contains your current visa details. Btw, you don't want your employer to generate a new CoS so early as your visa is valid until November. Sponsor will need to apply for COS before the 5th of each month and decisions are made around 11th/12th of each month.

Are you in country still with your initial cos or have you extended in the meanwhile ?
if your current leave is based on your initial COS, assuming your leave hasn't expired yet, you should be using your Initial leave- COS number.
If you have extended, you should use your latest COS number.

Sponsor should notify the Home office of any significant salary increase, unless it's a regular Annual pay correction / increase.

Assume, if home office has any concerns about the % of hike, they would question the sponsor for valid reasons.

Applying to change from a dependent to tier 2 general for the first time definitely has to be Unrestricted COS and you will have to apply for entry clearance from home country.

Employer has the ability to request both restricted and unrestricted COS.

When you apply for PBS dependent visa, the dependents are issued their visa in line with Primary applicant's existing visa end date.
So, in your case, it has to be upto 3 years as your current leave expires in 14 months.

You are permitted to change from a PBS dependent visa to a Tier 2 General Visa.
However, you will need a sponsor
Sponsor will need to do RLMT
Raise a Restricted COS

You will have to apply for Entry clearance from your Home country.
