Best to travel with her marriage certificate to be on safer side. The spouse visa that she came to UK will have the name of her husband endorsed on it, so there won't be any confusion when being cleared by immigration officials.

You need to re-apply, this means you have to submit fresh application with all relevant documents you sent the first time, you will mentioned in your supporting statement that you applied before but was refused because of the reasons you gave and even went through appeal which was refused. The best bet is to contact Norwich Council, you can change the venue of the ceremony to your preferred one, but you will first contact the Home Office to change the venue, then Norwich Council will send your Naturalization Certificate back to them, they will now send it to your new preferred venue, ...

What is required from you is proff of photo ID i.e full U.K. driving licence and letter that was sent to you confirming approval of your naturalization, just these tow are fine.

It has been said so many times on this forum that IELTS expired result is accepted for Naturalisation applications. You can go to British Citizenship/Naturalisation forum to read more, even Home Office did confirm it when someone sent an e-mail to ask this question.
