What documentation is required for an Indian Entry X Visa application?

Does one need to submit a copy of all the pages of the former Indian passport or would a copy of the surrender certificate be sufficient or are both required?

IMHO - In My Honest Opinion. It is a textspeak abbreviation. If your second child is born in the UK, s/he will be British by birth. Self attestation means signing on each sheet. No stamps required. I have been advised to write the date next to the signature as well, but that is not technically a part of the self-attestation.
