That's correct . That is what is your salary and should meet salary requirement. The amount which your getting while on maternity leave isn't your salary. The booking fees as far as I know is not extra. It is part of the application form fees so they are actually taking 100 pounds in advance and during the actual payment the 100 pounds will be adjusted. I have also done my appointment last week and was not asked for booking fees.

First of all you don't need last 12 months payslips. You can carry payslips for all last five years if you want to be in safer side.
