There is no such condition for dependent ILR. Abscences should be limited and for good reason. Why was she out between Jan 2012 and Sep 2012 ? If it is related to maternity , medical reason etc.. then you can apply now itself.

She can be on PBS dependent VISA until she qualifies for ILR.

You can apply up to 28 days before 25 jun 2014 . For PEO application you need an extension. You can apply in post and vary . You can search forum on varying application.

180 days is for each qualifying year. So , you are fine.
If you can't vary your application or apply ILR in post then you need another extension.

Correct . It is required in SET(O) forms . This observation is based on forum member experience . Till now i haven't come across any case where CW insisted on the letter. Anyway , for the people who can easily get it should have it.

Based on forum member experience letter isn't required for absences <=180. If you can get letter easily then go for it .Take print out of PBS for english language requirement . That is suffice.

If your abscences are <=180 days in each of the qualifying year you don't need any letter. If you still want to write one , You can write a personal letter detailing it was paid leave and that will be suffice.

It is quite difficult unless you have extremly good relationship with employer and also you give a fairly long time commitment to stay.

Job title isn't important . You need to choose a SOC code that best describes your job . Also , it is for tier 2 route only and your employer should provide you all required information in this section. There is absence of 8 months (which breaks into 2 calculated years making less than 180 days in each)--> As long as your abscence is <=180 days in each qualifying year it is fine . Generally , you don't need any letter related to it. But as you have got already one there is nothing to worry about.

As you have travelled within 90 day of EC date you can count your qualifying period from feb 15.
You don't need an extension you can apply up to 28 days before 15th feb 2015 i.e. from your fifth anniversary.

90 days concession rule is from entry clearance date .
Regarding your entry date, in my opinion 2nd december would be considered . But please wait if someone has to suggest otherwise.

For PBS dependent ILR you need cohabitation proof since last VISA extension only. So, you are fine.
Normally , for limited paid annual leaves you don't need any letter . If you can get it easily go for it.
Your wife still needs to give LIUK test.
