The letter said my application for extension was considered under the terms of the hsmp judicial review. But this doesn't matter. You are covered by JR. The JR applies to those who applied to the HSMP before 7 Nov 2006 . So what counts is your application date, not your approval date. Anyway, it is a year and a half away. And by then this government might change the qualifying time for ILR to 6 or 7 years so I wouldn't lose sleep over those 10 extra days and added fees you need. Why can't you just stay on Tier 1 and extend it before Sep 2012? Are you worried you won't meet the points requirement? Otherwise, there is no advantage in switching.

The OP is covered by HSMP Judicial Review (5 years). He needs to fill up 9H.

9L is for Tier 1, not original HSMP. And the question you are all referring to is for people who switched from other categories to Tier 1, like WP. This does not apply to the OP.

Do not send bank statements, salary slips etc.

Stick to the list that's been given by another poster. This is not a visa application, it's just a correction of the visa expiry date. So no need for those other documents. Keep those safe, you'll need them for ILR.

It is possible. But it is like biting your own foot because you will definitely need to do 2nd extension as the new rule laid to case worker states them to issue visa from date of application if applied earlier than 28 days rather than expiry date of visa. though applying 1-2 month earlier is fine....

Academic reference is a 1-page letter confirming the info stated in your post. Academic transcript can be several pages, showing every term/semester/trimester you were enrolled, courses you took and the grade for each course. At the top, it will have a few lines stating your name, degree awarded etc.
