Where does it say in documents that EU citizens who work in UK (but do not have permanent residence yet) cwill still be able to use NHS for free? Visa-waiver cannot be changed to any other status once inside USA, or extended beyond 90 days (the only exception - if you marry US citizen). So yes, it is absolutely a MUST for European citizens to get H1B visa stamp in the US embassy abroad before you head for USA.

In order to be able to apply for Family Permit, they have to either
- be financially dependent on you/your husband (rely on your money to satisfy their essential needs)
or
- live with you as members of your/your husband's household immediately before you moved to UK (if this was a recent move)
