Hi,
You should just write ur name and date of birth behind the photos.
Referee doesn't have to write anything there
This is just in case if your photo gets detached and it can traced back to the original application.

But I did not have any of these documents to submit. so, I asked her to go ahead and submit the
application. Do you think HO will ask me for more documents if they want to see or will they
reject my application outright?

It should be fine as you can get multiple copies of the birth certificate from registrar office
and they are all accepted if signed by the registrar as true copy of the original entry in the
births register.
