I do not think they change the form every month .. You can fill the form and just check the government site again in the 1st week to check if they have changed the version.. and do the check again 2 days before you go for your appointment.

I had also used IELTS for my Tier 1 application in 2009 (IELTS test was given in Nov-2008).

He was eligible the day he was born. You could have applied for his passport. I think you will now have to fill in form MN(1) and register him as British National.
Even as per my understanding, you cannot go for ILR through this visa (ICT-Long Term). This ILR route (ICT), closed on 6-Apr-2010 and you have come in 2013 so, I do not think you are elegible. You can still check with solicitor though ..
