If she came in April 2011 on a Tier 1 Dependent visa then she has been eligible ever since April 2013.

Tier 1 Dependents who applied after 9th July 2012 have to wait for 5 years and in your case she just had to wait for 2 years which is already over in April 2013

I am Tier 1 General, so I am not sure about specific requirement for Tier 2 General, but otherwise it looks fine.

I have written "Bank Statements from March-2014 to May-2015" in that section

For cohabitation documents also, I have written in a descriptive way.

Thanks a lot guys for your concerns.

I would submit from March-14 to Feb-15. Even if they exclude my March-14 salary, I am way above the mark so not much of a concern.
